In patients with rheumatoid arthritis, all TNF-? inhibitors prevent damage of bone and cartilage more effectively than methotrexate alone. Combination therapy with methotrexate and a TNF-? inhibitor also has consistently been shown to be more effective than single-agent TNF-? inhibitor therapy. Therefore, these agents are often administered concomitantly with methotrexate.

The TNF-? inhibitors etanercept, infliximab, and adalimumab are effective in rheumatoid arthritis, and several more TNF-? inhibitors are anticipated to be released during the next few years. Etanercept is a genetically engineered human fusion protein; adalimumab is a fully human monoclonal antibody to TNF-?; and infliximab is a chimeric antibody specific for human TNF-?.The efficacy of infliximab is believed to decrease with repeated dosing unless methotrexate is administered concomitantly; this decreased efficacy is believed to occur because the patient develops an immune response to the chimeric antibody. Approximately 1% of patients treated with infliximab develop serious infusion reactions.
