Это единственный вариант представления, который мне пришёлся по душе (если кто назовёт его имя - буду премного благодарен!!) и заставил подумать:
- А почему нет??

A few months ago I presented to you this hypothetical case--for follow up scroll below...
Guys,
I will try to tell the story briefly. 55 years old man.
Some 3 years ago I fixed his left inguonal hernia with mesh (light mesh, plug and patch). Uneventful.
A year ago he developed a contra-lateral hernia and decides to have it fix in a hospital an hour away from here. Maybe thought he'll get a better deal there???
The surgeon did Lichtenstein (Marlex). According to patient immediately after the operation he started to feel a burning pain at the op site.
An incapacitating pain followed. Surgeon tried local injections of steroids and marcaine....pain clinic tried injecting/blocks to no avail.
Sent to me for 2nd opinion. Seems suffering a lot constantly from unbearable pain.... Less pain when reclining backwards. Tender elicited by tapping overlying the scar at pubic tubercle level. The guys bags me to do something.
I offered: inguinal 3 nerve neurectomy through the scar. Told him that at least part of old mesh may have to come out. May have to insert new mesh. Warned him that if groin frozen may have to remove the cord and testicle.....
Offered to send him back to the other surgeon or to a COE but now it seems that he likes me...

Well, I operated on him 3 weeks ago.
Attached is a copy from the op report (sorry for the few spelling mistakes--this is an uncorrected draft).
The report may sound to you confusing--the anatomy and the previous repair were initially confusing and surprising to me...
Pathology confirmed that I resected a few segments of nerve.
On follow up today: patient is cured. No pain whatsoever. Off the pain medication he was swallowing for more than a year. Testicle OK. Decreased sensation inner aspect of thigh and scrotum.
A few months ago the patient entered my office groaning and grimacing from pain, not being able to extend his thigh and today he is happy and smiling. Quite gratifying I must say--as if a laser has cured him ..
